The Kelley & Gardner Orthodontics team is proud to give back to our community through sponsorships and donations. Our doctors value education and love to support local schools, sports teams, and other organizations in our community.

Each year, Kelley & Gardner Orthodontics receives hundreds of donation and sponsorship requests, and unfortunately we do have a budgetary limit for these gifts. Our goal is to support philanthropic causes which primarily benefit schools or the community at large. As a result, requests for sponsorships of private or travel sports, pageants, mission trips, etc, may not be approved for funding. In addition, priority will be given to patients who are in active treatment and have an account in good standing.

To request consideration for a donation or sponsorship, this form must be completed and submitted AT LEAST 4 WEEKS before the deadline. Incomplete forms could disqualify the application from consideration.
